A letter to my loyal patient,
It has been my privilege to participate in your healthcare over the past several years. After much reflection, I have made the decision to retire and close my medical practice in Cranston and Smithfield effective October 31, 2026.
As I conclude this chapter of my career, I want to ensure you have the information needed for your continued care. As many of you know, my practice is part of Southern New England Healthcare for Women, also known as SNE Women’s Health. If you choose to remain with the group, your medical records will be made available to my colleagues at SNE Women’s Health. Please refer to the enclosed list of physicians and locations and visit snewomenshealth.com for additional information.
If you prefer to establish care with another provider outside of SNE Women’s Health, our office will be happy to assist with transferring your medical records. To do so, please complete a medical records release form and fax it to our office at 401-946-4077 before October so that we may facilitate the transfer in a timely and appropriate manner.
